A growing number of older adults have chronic illnesses that make it challenging to function at home. These situations can lead to isolation, loneliness and depression. Long-term support may be needed to improve their health and well-being so they can remain living independently.
Our Suncoast PACE (Program of All-Inclusive Care for the Elderly) is available for Pinellas County residents who are ages 55 and older with chronic health conditions and certified as needing nursing home-level care, but can still live safely in the community. Participants greatly benefit from our comprehensive care and support services that help keep them healthy, active, social and in their own homes for as long as possible.
The Suncoast PACE Center has a medical clinic for primary care, specialized care and therapies, as well as a day center with nutritious meals, special events and stimulating recreational, fitness and educational activities. Other services include social services, transportation and in-home care and assistance, as needed.
Health & Pain Management
Angela Dowding, ARNP (Advanced Registered Nurse Practitioner) started working at the clinic earlier this year. She is one of many team members providing expert and compassionate care to enhance comfort and quality of life for participants and their families.
In her role, Angela regularly assesses and manages participants’ diseases, pain and symptoms.
“I get to know our participants well. Most of the time, I see them for their acute conditions and six-month evaluations. I provide treatment, make referrals, and review labs and other care by our team. We work to keep participants’ diseases under control so they don’t undergo any unnecessary hospitalizations,” Angela said.
Caregiver Education
Another major part of her care is educating family caregivers about their loved ones’ health.
“I also work with families. We had one gentleman whose wife has moderate stage dementia. He came in twice a week and I went over everything with him. I try to make sure that caregivers like him are doing well so they can continue providing the best possible care.”
Collaborative Team Care
Angela enjoys the environment at the clinic and sees great value in its care.
“I used to work in a big trauma unit at a hospital and didn’t really know everybody. It’s nice at Suncoast PACE to have regular team meetings, communication and access to be able to evaluate our participants. Our staff and volunteers are wonderful and our participants are very sweet. I like what I do and our all-inclusive model. We coordinate everything for participants, making it easier for them and us to get good results,” she said.
